Repeat Prescription Request

We issue 28 days supply of medication on each occasion.

To order your repeat medication please complete the form below, or place a written request at surgery reception. Email confirmation of web orders will be issued once the prescription has been printed - this may take some time so please be patient. (Email delivery can take up to 12 hours & orders will only be processed between 10am & 3pm) Thanks.

Please allow two clear working days for your prescription to be issued by the surgery.

The chemist will require an additional two working days to dispense your prescription.

You can use the printed list supplied with your prescription to re-order

your medication next time.

If the order is urgent, please speak to one of our dispensary staff.

order. Please enter YOUR FIRST FORENAME ONLY (adding a middle name or initial will prevent the system from recognising you!). When adding your date of birth you need to change the year first.

The surgery offers a free delivery service for dispensing patients who cannot get to the surgery.

This is on two days a week on a Tuesday in Horning, Neatishead and Tunstead and on a Thursday to Salhouse and Rackheath.
